Awards Nomination Form

The Mount Mary Alumni Awards nomination form offers nominators the chance to share more about the person they would like to honor. With your nomination form, please include at least one (1) letter of recommendation, as well as any supporting materials to be referenced during deliberations.

*Supporting materials can include award recognition, news articles/stories documenting their successes, letters of support from colleagues, supervisors, or peers, and any other documentation that will aide the committee in fully understanding their accomplishments.
